What is Housing Stabilization
Housing Stabilization Services (HSS) are a vital support system aimed at assisting individuals, particularly those with disabilities, in securing and maintaining stable housing situations. These services encompass a range of interventions and supports tailored to address various housing challenges individuals may encounter. HSS includes assessments of functional needs, assistance with housing transitions such as searching for accommodation and moving logistics, and ongoing support to sustain tenancy. Additionally, HSS provides education on tenants' rights and responsibilities, advocacy to prevent eviction, and the development of crisis plans to address potential housing emergencies. Ultimately, the goal of HSS is to empower individuals to achieve and maintain stable housing, thereby promoting overall well-being and preventing homelessness or institutionalization.

Eligibility
- Be 18 or older and on Medical Assistance (MA).
- Have a documented disability (aging, blindness, Title II disability, injury, illness, developmental disability, mental illness, transitional care, or learning disability).
- Need assistance in communication, mobility, decision-making, or managing challenging behaviors.
- Show housing instability (homelessness, transitioning, or at risk).
- Be at risk of institutionalization, meeting care level criteria, and eligible for specific waivers (BI, CADI, CAC, DD, EW)
